Treatment with Transjugular Intrahepatic Portosystemic Shunt(TIPS) in Esophageal Variceal Bleeding

Abstract
Purpose : To report our experiences of transjugular intrahepatic portosystemic shunt(TIPS) for the control of variceal bleeding.
Material and Methods : Five patients underwent TIPS creation for variceal bleeding. All patients are male with liver cirrhosis. All patients have been treated with multiple endoscopic sclerotherapy or ligations for multiple episodes of bleeding
and
after the procedure, and all patients were followed up at regular interval after TPS creation.
Results : Shunts were successfully completed in all five patients. No serious procedure-related complication was noted. Portal vein pressure was reduced from 32.2 mmHg to 22.2 mmhg. The residual portohepatic pressure gradient was 7.6 mmHg. Two
patients
developed hepatic encephalopathy at seventh and tenth day after the procedure. During the follow-up period(3-7 months), all patients have survived, and recurrent bleeding was not developed in any patient.
Conclusion : TIPS is effective in lowering portal pressure and controlling variceal bleeding, particulary in patients with acute variceal bleeding unresponsive to the sclerotherapy and patient contraindicated for surgical shunt operation.
